Using the Request for Accommodations Form in Accommodate
- Navigate to the Accommodation Request for Accommodations Form. Once there, please complete all fields marked with an asterisk (*). Select Student from the role dropdown menu. Students then must include their name, D#, university email address, and cell phone number.
- Click the “Accommodations Being Requested” field. Please enter the type of accommodation/accommodations you are requesting. If you are unsure, you can leave it blank.
- If you would like information on specific documentation requirements, click “Please click here to review the documentation requirements.” This will take you to the documentation requirements webpage.
- To add your medical documentation, click the “Add Item” button. This step is not required as part of the application process. Documentation can be emailed directly to the CDR at cdr [at] desu.edu.
- Title your document, upload your file, provide a brief description, and click Save & Close. You will receive a submission confirmation pop-up when complete.
